Trial Class Registration

To register for a Trial class, please take the time to fill out the information below.
One Trial class per technique will be allowed per student.
Please note, a Full Student Registration and a $20 non-refundable Registration Fee, per family, must be submitted to finalize your students Full Registration into your selected class(es).

I understand while participating in this activity, I may be photographed and/or videoed.  I agree to allow my student(s) photo, video, or film likeness to be used for any legitimate purpose by the activity holders, producers, sponsors, organizers, and assigns of St. Johns Stage.
